They are integrated into our Adult Home Care team and will help you and your family connect with local, community-based providers to ensure that you have the supports you need to achieve the best possible quality of life.

Our staff starts by by getting to know you. Their goal is to understand your values, preferences, and needs. Supports usually include, but are not limited to, care coordination, patient and family counseling, psycho-social assessments, goal setting, education, advocacy, and, help with resource planning, including filling out advance directive paperwork. As needed, they can also help connect with you with resources to help pay for care.
