Our Palliative Care program offers medical care, education, and emotional support at home to Central Vermonters living with serious illness. Our team of expert clinicians and caregivers, led by Dr. Kelley Elwell, DNP, APRN, FNP-BC, will help you manage your pain and symptoms, offer compassionate guidance on end-of-life care options and setting up an advance-care plan, and connect you with resources.
We want you and your loved ones to get the support you need to experience the best-possible quality of life when it matters most. Dr. Elwell is available when you are to answer questions and help you take the next step.
