Central Vermont Home Health & Hospice (CVHHH) is seeking a full-time, Home Care Clinical Manager. CVHHH is a full service, not-for-profit Visiting Nurse Association committed to providing high quality, medically necessary home health and hospice care to all central Vermonters, regardless of a person’s ability to pay. In addition, we promote the general welfare of all central Vermonters with health promotion, long term care, and maternal-and-child health services.

Your Day-to-Day – Home Care Clinical Manager
- Supervise an interdisciplinary team to bring hospital-level care to people in their homes and provide case management to ensure our patients’ needs are met.
- Collaborate with your team, colleagues, local physicians, and community providers to provide exceptional health care to our patients.
- Participate in ongoing strategic planning and management initiatives.
- Engage with patients and family members to provide in-home high-quality, cost-effective care.
- Support staff toward the goal of optimal patient care and satisfaction.
- Ensure patient care is provided according to CMS regulations, Vermont Home Care Designation Rules, and CVHHH policies and procedures.

Requirements – Home Care Clinical Manager
- VT Licensed RN (BSN preferred)
- A minimum of (5) years working experience in the qualified professional with at least three (3) years of community health experience and at least two (2) years of supervisory/management preferred.
- Valid driver’s license
Minimum Starting Salary $90,000 with a $10,000 sign-on bonus and up to a $3,000 relocation bonus.
