Who We’re Seeking: We are seeking a Home Health Clinical Manager who is skilled at planning, organizing, and implementing the day-to-day operations of a busy home health program, ensuring that your staff is supported to achieve optimal productivity. With strong time management skills, you can meet important deadlines while emphasizing the importance of communication and follow-through as key pillars of effective management. Your solid problem-solving and decision-making abilities help guide the team through challenges, and you motivate others by fostering a work culture that prioritizes employee development. You combine a strategic and creative mindset with a calm and positive demeanor to lead the team to success.
Your Responsibilities: As a member of our dedicated team, you’ll:
- Supervise an interdisciplinary team to bring hospital-level care to people in their homes and provide case management to ensure our patients’ needs are met.
- Collaborate with your team, colleagues, local physicians, and community providers to provide exceptional health care to our patients.
- Participate in ongoing strategic planning and management initiatives.
- Engage with patients and family members to provide in-home high-quality, cost-effective care.
- Support staff toward the goal of optimal patient care and satisfaction.
- Ensure patient care is provided according to CMS regulations, Vermont Home Care Designation Rules, and CVHHH policies and procedures.

Requirements
- VT Licensed RN (BSN preferred)
- A minimum of (5) years relevant professional experience with at least three (3) years of community health experience and at least two (2) years of supervisory/management preferred.
- Valid driver’s license
