Our Diabetic Telemonitor Project is featured in Central Vermont Medical Center‘s (CVMC) 2015 Annual Report.
Take a moment to read the story, which demonstrates the value in working together as a system of providers to achieve shared goals–in this case, to care for at-risk, diabetic central Vermonters who had reported frequent hospitalizations or emergency room visits.
Patients were given telemonitors to monitor their vital signs, including glucose levels, and care was provided by a coordinated team that included a CVHHH nurse-case manager, a Community Health Team coordinator, a Diabetic Educator from CVMC, and a mental health case manager. We are happy to report that this collaboration helped 86% of diabetic clients stay out of the emergency room.
In partnership with CVMC, and all of our community providers, CVHHH remains committed to working together to support the health and wellness of all central Vermonters. For us, this means continuing to provide comprehensive, acute care in the place where you want to be — home.
