Adult Home Care

Our Adult Home Care program provides intermittent, short-term, medical care, education, and support as you recover from a hospital stay, injury, or surgery or if you need help managing your chronic disease. Services are provided in the comfort and privacy of your home by an interdisciplinary team of experienced, licensed nurses, physical, speech, and occupational therapists, social workers, and licensed nursing assistants.

You are at the center of what we do, and the care that we provide is guided by your unique needs and goals. Skilled Nursing

Nursing care is the foundation of our Adult Home Care program. Our team of experienced, licensed Home Care Nurses delivers hospital-level medical care to you in the comfort and safety of home. We offer a range of services to help you achieve optimal health and wellness and maintain your independence.

Rehab Therapy care

We provide physical, occupational and speech therapy care in the safety and comfort of home. Our team of experienced, licensed clinicians will work with you to heal after a fall, surgery, injury, or other incident. We are here to help you regain your strength and mobility and maintain your independence so that you can get back to doing your favorite activities.

Licensed Nursing Assistant

Licensed Nursing Assistants, or LNAs, are the heart and soul of our Adult Home Care program. LNAs work side-by-side with our Home Care Registered Nurses and Rehab Therapists to ensure that you are safe and comfortable at home.

Medical Social Workers connect you and your family members with community resources and services that will help you meet and sustain your goals, stay in your own home, and maintain your health, wellness, and independence.

Telemonitors are easy-to-use, portable electronic devices that let us provide supplemental at-home support and remote, daily monitoring for people living with high-risk chronic diseases or who are at risk of hospitalization. Telemonitors help us better help you manage your condition, avoid unnecessary trips to the hospital, and stay safely in your own home.
