CVHHH’s Palliative Care Consultative Service offers pain and symptom management, education, advance care planning, and emotional support for Central Vermonters living with serious illness in collaboration with their clinical team. We will meet you in the comfort of your home and can provide guidance on end-of-life care options and community resources that align with your goals of care.

Palliative care is not limited to a specific diagnosis, stage of illness, or life expectancy. It can begin at any point after a serious illness diagnosis and continue alongside treatment from your primary care provider or specialist, adding an extra layer of support focused on your comfort and quality of life.

We want you and your loved ones to get the support you need to experience the best-possible quality of life when it matters most. We are here to answer questions and help you take the next step.

What palliative care includes

Pain and symptom management

Our team helps manage the pain, discomfort, and other symptoms that can come with serious illness, so you can stay as comfortable and active as possible. We work alongside your primary care provider or specialist to adjust your plan of care as your needs change.

Advance care planning

We help you think through and document your wishes for future care, including your goals, values, and preferences for treatment. Having these conversations early means your family and care team know how to support the care you want, whenever decisions need to be made.

Education and guidance

We take the time to explain your diagnosis, treatment options, and what to expect, so you and your family can make informed decisions with confidence. We can also help you understand community resources and services available in Central Vermont.

Emotional and spiritual support

Serious illness affects more than the body. Our team provides emotional support for you and your family, and can connect you with spiritual care and other resources that support your overall well-being.

Paying for palliative care

We accept payment for our Palliative Care Consultative Service from Medicare, Medicaid, workers’ compensation, private insurance, and private pay. If you are paying out of pocket or are uninsured, we will provide a good-faith estimate of what you may be charged, at your request or before care begins.

How to start palliative care

We serve residents in 23 towns in Central Vermont across Washington and Orange Counties, from Montpelier, Middlesex, and Moretown to Duxbury and Worcester.
