CVHHH Recipient of Grant Funding to Expand Telehealth Services

Thanks to the generosity of the Connectivity Care Packages Pilot Program (CCP), with Vermont Program for Quality in Healthcare (VPQHC) and Vermont Department of Health (VDOH), CVHHH purchased tablets and Wi-Fi boosters to expand its telehealth program. The purpose of the CCP Program is to support equitable access to telehealth services during COVID-19.

CVHHH has offered telehealth services for many years. During the pandemic, CVHHH expanded the program to meet patients’ needs and minimize possible transmission of COVID-19. Pictured are Katy Leffel, RN, IBCLC, Maternal-Child Health Manager, and Kelly Bishop, Chief Financial Officer.

How CVHHH uses telehealth

Clinicians and caregivers across CVHHH programs, from Maternal-Child Health to Hospice, utilize telehealth to support the medical and emotional needs of Central Vermonters of all ages.

  • COVID-positive patients can connect with their CVHHH clinician and primary care provider via video visit while they are at home recovering
  • CVHHH clinicians can answer client questions and provide education on a range of topics (i.e. proper inhaler use, taking vital signs, performing dressing changes, etc.) between in-person visits. This helps patients better manage their conditions and improves confidence and independence that lasts long after a person is discharged from our care.
  • Personal Care Attendants can facilitate video visits for their clients from home using iPads to connect with primary care providers.
  • Hospice nurses can connect with patients and caregivers to assess a person’s needs in real time and make recommendations to help better manage a patient’s symptoms and pain. An in-home visit may be indicated, and a plan can be initiated following a video assessment so that time is maximized while the clinician travels to the patient at home. This puts caregivers at ease and improves collaboration between Hospice staff and caregivers.
  • Maternal-Child Health nurses provide lactation education and support and conduct child development screenings.
  • Support patients’ emotional needs and reduce the likelihood of social isolation.
