Central Vermont Home Health & Hospice is a full service, not-for-profit Visiting Nurse Association committed to providing high quality, medically necessary home health and hospice care to all central Vermonters, regardless of a person’s ability to pay. In addition, we promote the general welfare of all central Vermonters with health promotion, long term care, and maternal-and-child health services.

Who You Are – High-Tech Nurse

• You appreciate autonomy, flexibility, and are comfortable working in a homecare environment.
• You demonstrate excellent assessment skills and are knowledgeable in the management of tracheotomy, G-tube and ventilation needs. You also demonstrate the ability to complete accurate and timely documentation in the EMR.
• You enjoy providing high tech, one-on-one care, and consistency in your work.
• You are looking for some work/life balance, this is a per diem position.

Your Day-to-Day – High-Tech Nurse

• Collaborate with the client and caregiver to set up a care schedule that works for you and them. Notify your manager of changes in your schedule.
• Provide High-Tech Nursing visits to included medication management, care of tracheotomy, vent, and G-tube, head to toe assessment, vitals, personal care, and respite.
• Document in the EMR at each visit and communicate any changes to the care management team as needed.
• Attend required meetings or trainings as scheduled.

Requirements – High-Tech Nurse

• VT Licensed LPN or RN (BSN Preferred)
• Valid driver’s license

• Minimum of one year of experience in medical/surgical or High-tech environment
