CVHHH is dedicated to providing home health care and supportive services to Central Vermonters at home. This work is possible through the generosity of donors and volunteers who contribute funds and time to help bring CVHHH’s mission to life. When you make a contribution to CVHHH, you are directly supporting the health and wellbeing of your family, friends, and neighbors.

  • Cover costs for ambulance transport, durable medical equipment such as lifts for patients, and medications for hospice patients.
  • Provide community education, volunteer training and support, and grief support groups.
  • Deliver lactation and nutritional support, skilled nursing care, and one-on-one guidance to Central Vermont children and families.
  • Support safety and independence for Central Vermonters living with chronic conditions.
  • Offer daily monitoring via a telemonitor tablet, visits, and care management and coordination from a clinical care team.
