CVHHH’s Palliative Care Consultative Service offers pain and symptom management, education, advance care planning, and emotional support for Central Vermonters living with serious illness in collaboration with their clinical team. Dr. Kelley Elwell, DNP, APRN, FNP-BC, our Palliative Care Nurse Practitioner, will meet you in the comfort of your home and can provide guidance on end-of-life care options and community resources that align with your goals of care.
We want you and your loved ones to get the support you need to experience the best-possible quality of life when it matters most. Dr. Elwell is available when you are to answer questions and help you take the next step.
