We’re here to help your family thrive

At CVHHH, we believe that everyone deserves to have the best pregnancy, birth, and parenting experience possible, and the care we provide for parents and babies supports this belief. Our Maternal & Family Child Health (MFCH) program provides a range of medical and non-medical supports and services to promote maternal and child health and wellness from pregnancy through birth and beyond.

MFCH Registered Nurses are reliable and knowledgeable, and they understand the range of emotions that come with the experiences of pregnancy and becoming a parent. We know that the benefits of early intervention last long into a person’s life. Through every step of the journey, we are here to ensure that parents, children, and families have the support they need to thrive.

CVHHH is a proud partner of Strong Families Vermont Home Visiting Program. Watch this video about the program, which features one of our MFCH Registered Nurses:

How to start care

We serve residents in 23 towns in Central Vermont. If you live outside of our service area, please visit VNAs of Vermont to find the agency in your region. Care is most often initiated through a referral from your physician. You can ask to be referred to CVHHH specifically. Once your referral is processed, you will be contacted by a member of our intake team.

The Palliative Care Consultative Service does not require a physician referral to initiate care; patients can self-refer for this service.
