Bringing care to your front door

We know that support is often needed to make it possible to continue to live safely in your home. Sometimes, all that’s needed is help getting back on your feet after surgery or as you recover from an illness. Or, maybe you need help caring for a loved one who wants to continue to live at home and needs support with activities of daily living.

Our Long-Term Care team provides in-home personal care and case management for adults living with disabilities and for older Vermonters who want to continue to live at home.

If you meet the following criteria, you may be eligible to receive Long-Term Care services:

  • You are a Vermont resident
  • You are at least 65 years old, or at least 18 years old with a physical disability
  • You meet the financial criteria
  • You meet the clinical criteria for nursing-home level of care, including having a physically-limiting condition resulting from a stroke or dementia, or something similar, or as a result of aging. Clinical criteria is determined by a nurse employed by VT Department of Aging & Independent Living (DAIL).

We serve residents in 23 towns in Central Vermont. If you live outside of our service area, please visit VNAs of Vermont to find the agency in your region. Care is most often initiated through a referral from your physician. You can ask to be referred to CVHHH specifically. Once your referral is processed, you will be contacted by a member of our intake team.

The Palliative Care Consultative Service does not require a physician referral to initiate care; patients can self-refer for this service.
