The CVHHH Adult Home Care program provides intermittent, short-term, medical care, education, and support as you recover from a hospital stay, injury, or surgery, or if you need help managing your chronic disease.
You are at the center of what we do, and the care that we offer is guided by your unique needs and goals in partnership with your doctor. Services are provided in the comfort and privacy of your home by an interdisciplinary team of experienced, licensed nurses, physical, speech, and occupational therapists, social workers, and licensed nursing assistants.

Adult Home Care services
Skilled Nursing
Our team of experienced, licensed Home Care Nurses deliver hospital-level medical care to you in the comfort and safety of home. We work closely with your medical provider to ensure that you are getting the care you need to heal and regain your strength, maintain your independence, and experience the best-possible quality of life.
Rehab Therapy
Our Rehab Therapy team provides a range of Physical, Occupational, and Speech Therapy services to Central Vermonters at home. We work in partnership with your medical provider and with the other members of your CVHHH Home Care team to help you set and reach meaningful goals and to regain your strength and independence and get back to doing routine activities of daily life.
Licensed Nursing Assistant
Licensed Nursing Assistants, or LNAs, are integrated into our Adult Home Care team and work under the supervision of a Home Care Registered Nurse. LNAs provide hands-on personal care including bathing, dressing, toileting, and grooming. They also can provide medication reminders, and offer support with safe exercises at home.
Social Work
Social workers are trained problem-solvers with extensive experience in the healthcare sphere. Our staff starts by getting to know you. Their goal is to understand your values, preferences, and needs. They are integrated into our Adult Home Care team and will help you and your family connect with local, community-based providers to ensure that you have the support you need to achieve the best possible quality of life. As needed, they can also help connect with you with resources to help pay for care.
Telemonitor Services
Telemonitor services are a way of providing remote care, support, and education to Central Vermonters of all ages at home via the use of hand-held tablets equipped with video and audio functionality. Especially valuable for Central Vermonters living with high-risk chronic conditions, patients can use their telemonitor tablet to communicate with their CVHHH clinician, primary care provider or specialist, and with family members, and we can keep close track of your vital signs and catch potential exacerbations before a visit to the emergency department is required.
How to start care
We serve residents in 23 towns in Central Vermont. If you live outside of our service area, please visit VNAs of Vermont to find the agency in your region. Care is most often initiated through a referral from your physician. You can ask to be referred to CVHHH specifically. Once your referral is processed, you will be contacted by a member of our intake team.
The Palliative Care Consultative Service does not require a physician referral to initiate care; patients can self-refer for this service.
