How to start care
We serve residents in 23 towns in Central Vermont. If you live outside of our service area, please visit VNAs of Vermont to find the agency in your region. Care is initiated through a referral from your physician. We can help facilitate conversations about care with your physician. Once your referral is processed, you will be contacted by a member of our intake team.
Please note: The Palliative Care Consultative Service does not require a physician referral to initiate care; patients can self-refer for this service.
