About the Position
Who We Are
Central Vermont Home Health & Hospice (CVHHH) is seeking a full-time Clinical Preceptor. CVHHH is a full service, not-for-profit Visiting Nurse Association committed to providing high quality, medically-necessary home health and hospice care to all central Vermonters, regardless of a person’s ability to pay. In addition, we promote the general welfare of all central Vermonters with health promotion, long term care, and maternal-and-child health services.

Your Day-to-Day
- Educate, evaluate, and mentor new clinical staff through orientation and onboarding.
- Implement and reinforce clinical practices utilizing the Vermont Nurse Practice Act.
- Observe clinical skills and patient interactions and provide feedback to foster the development of staff.
- Collaborate with the Quality Nurse Educator on education development, communication, best-practice, and emerging home care trends.
- Review clinical documentation and provide guidance to new and current staff on accuracy and completeness.
- Assess new clinicians on a continuous basis to determine clinician understanding and need for re-education.
- Meet with Quality Nurse Educator and Clinical Managers to discuss progress of new clinical staff.
- Participate and assist with professional development activities of the multidisciplinary healthcare team.
- Provide shadow experiences for prospective clinicians interested in working for the agency.

Requirements
- Graduate of an accredited nursing education program with a BSN.
- Minimum of two years’ full-time clinical experience.
- Home Care experience and/or previous case management experience preferred.
- Clinical Preceptor experience preferred.
- Valid driver’s license
