In light of the COVID-19 pandemic, we made changes to how we provide care and screen patients and staff to ensure the safety of our community. We are open for business and continue to accept referrals and care for current patients for all our specialty programs, including wound care, telemonitoring, homesight (for individuals with low vision), and falls. To learn more about how we can serve you, contact Kim LaGue using the contact form on the right side of this page. To read more about our COVID-19 response and updated protocols, visit this page on our website.

We offer a number of specialty care programs that can improve your quality of life, reduce the need for emergency care and provide valuable tools and resources so that you can remain safely at home. We continuously invest in technology and clinician training so that we can provide the best care possible, including for complex conditions such as slow-healing wounds or reduced vision.

We believe that effective use of technology–and knowledge–can improve the hands-on care of our nurses and therapists and will provide you with a better health care experience at home.
