Nurses to Oversee Clinical Home Care Teams

Angie Demasi, RN, and Shelby Lunn, RN, WCC, have been named Interdisciplinary Clinical Team Managers. Together, Angie and Shelby oversee about 30 visiting home care clinicians who care for residents in the 23 communities CVHHH serves. Home care staff are divided into two teams—based on geography—made up of Registered Nurses, Physical, Occupational, and Speech Therapists, Social Workers, and Licensed Nursing Assistants. Angie and Shelby, who have almost 30 years combined nursing experience, provide critical leadership, guidance, education, and support for staff who are on the road visiting patients.

Angie DeMasi has worked for 15 years as a Registered Nurse in a range of care settings, including as a visiting Hospice & Palliative Care Nurse at CVHHH and as a Medical/Surgical Nurse at University of Vermont Health Network-Central Vermont Medical Center (CVMC). Angie, who is a Certified Hospice & Palliative Care Nurse, helped start the Palliative Care Department at CVMC. She also helped found the Medication Reconciliation Department at CVMC. Angie looks forward to working together with her staff to build a strong sense of teamwork and collaboration. Angie grew up in West Berlin and graduated from U-32 High School. She received her clinical training at Vermont Technical College and University of Vermont. She lives on a small farm in Northfield with her sons and husband.

Shelby Lunn began her nursing career 14 years ago as a Licensed Nursing Assistant and evening Nurse at Woodridge Rehabilitation & Nursing in Berlin. In 2012, Shelby joined CVHHH as a visiting nurse. Shelby is Wound Care Certified, which means she has received specialized training to provide advanced wound care to clients at home. Shelby has enjoyed making the transition from visiting clinician to manager and looks forward to utilizing her clinical expertise to support her team to provide a true continuity of care for patients. Shelby grew up in Orange and graduated from Spaulding High School. She received her nursing degree at Vermont Technical College. She lives in Roxbury with her daughter.
