Hours: This position will serve as a backup for weekends and holidays on a per diem basis. Weekend hours are approximately 5 hours daily, 9-2pm.

Location: Remote within Vermont

Who We’re Seeking: Are you passionate about empowering patients to maintain their independence while receiving quality healthcare? In this role, you will review and analyze vital signs and other data submitted by clients enrolled in our telehealth program. You must be able to identify trends and reports results to the Telehealth Coordinator in a timely manner. Experience in a home health setting or with telehealth is desired.

Requirements:

• Vermont Licensed LPN or RN
• Experience in medical/surgical preferred. Experience in home care and/or telehealth preferred
• Valid driver’s license
• HIPAA-compliant space with high-speed, dependable internet access
