Who We Are

Central Vermont Home Health & Hospice is a full service, not-for-profit Visiting Nurse Association committed to providing high quality, medically-necessary home health and hospice care to all central Vermonters, regardless of a person’s ability to pay. In addition, we promote the general welfare of all central Vermonters with health promotion, long term care, and maternal-and-child health services.

Your Day-to-Day

• Install telemonitor equipment in patients’ homes
• Provide training and education on the use of equipment
• Provide in-home services and complete tasks in the office
• Maintain telemonitor equipment properly, including cleaning and preparation for installation, resolution of transmission issues, maintenance of inventory
• Coordinate with telehealth manager, telehealth nurses, visiting staff, and patients.

Requirements

• Valid driver’s license
• High School Diploma or GED equivalent.
• LNA preferred (but not required)
