About the Position

We are looking for a Medical Social Worker (MSW) to support home care patients at home. In this role you will collaborate with members of our two interdisciplinary Home Care teams to ensure that your clients’ non-medical needs are met and that they have the supports in place to achieve optimal well-being and independence.

This role is eligible for a $8000 sign-on bonus

Minimum starting rate of $30-$35 hourly, dependent on experience.

Primary Job Responsibilities

  • Working in home care, you will have the unique opportunity to provide direct, one-on-one supports to individuals in a setting where they are most comfortable, at home
  • Develop trusting, meaningful relationships with your clients over extended periods of time – something that is not possible in a hospital or facility setting
  • Develop, communicate, and coordinate a goal-oriented plan of care in collaboration with the patient, their medical doctor, and family. This includes care plans for LNAs as needed
  • Travel in a designated territory within Washington County or an adjacent town
  • Maintain consistent, timely, and accurate communication with all members of the patient’s health care team
  • Keep up to date electronic documentation of care provided
  • Evaluate each person’s progress in meeting their goals of care and update plan of care accordingly
  • Evaluate evidence-based treatments for clients with psychosocial challenges that interfere in their ability to abide by medical plan of care

Job Requirements

  • Master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education
  • Minimum one year of progressive clinical experience in medically-oriented setting.
  • Exceptional new graduates will be considered
  • Ability to travel to clients within a designated territory
