About the Position

Who We Are

Central Vermont Home Health & Hospice is a full service, not-for-profit Visiting Nurse Association committed to providing high quality, medically-necessary home health and hospice care to all central Vermonters, regardless of a person’s ability to pay. In addition, we promote the general welfare of all central Vermonters with health promotion, long term care, and maternal-and-child health services.

Your Day-to-Day

  • Assist Choices for Care Clients in obtaining resources and services that they may be eligible for to ensure their ability to live at home
  • Provide in-home face to face visits with the client to monitor all services and changes in their needs.
  • Collaborate with community providers, such medical providers, Central Vermont Council on Aging, Washington County Mental Health, etc., to ensure clients’ needs are met, all while preventing duplication of services, unnecessary costs, and unnecessary administrative tasks
  • Monitor outcomes for the client as established through an effective assessment and current plan of care
  • Maintains client’s chart, including documentation of all waiver related services, assessments and updates, written notes of all relevant client communication and home visits, financial information necessary for continuation in the Long-Term Care program and necessary release of information forms

Requirements

  • Bachelor or Associates of Arts or Science degree; Certified as Case Manager by State of Vermont preferred
  • Two years’ experiences as a Choices for Care case manager preferred.  Two years’ experience in human services or nursing; or at least three years’ experience working with elders and/or adults with disabilities
  • Valid driver’s license
