Central Vermont Home Health & Hospice is a full service, not-for-profit Visiting Nurse Association committed to providing high quality, medically-necessary home health and hospice care to all central Vermonters, regardless of a person’s ability to pay. In addition, we promote the general welfare of all central Vermonters with health promotion, long term care, and maternal-and-child health services.

Your Day-to-Day – Home Care Clinical Lead Coordinator

  • Supervising an interdisciplinary team to bring hospital-level care to people in their homes and provide case management to ensure our clients’ needs are met.
  • Collaborate with your team, colleagues, local physicians, and community providers to provide exceptional health care to our clients
  • Participating in strategic planning and management initiatives
  • Daily engagements with clients and family members to provide in-home high-quality care timely in prevention of rehospitalizations
  • Ability to support staff toward the goal of optimal client care and satisfaction required. Knowledge of CMS regulations, Vermont Home Care Rules, and CVHHH policies and procedures.  Knowledge of and ability to comply with OASIS D, OSHA, Infection Control, HIPPA regulations required

Requirements

  • VT Licensed RN (BSN preferred)
  • A minimum of (5) years working experience in the qualified professional with at least three (3) years of community health experience and at least two (2) years of supervisory/management preferred
  • Valid driver’s license
