About the Position
Who We Are
Central Vermont Home Health & Hospice (CVHHH) is seeking a full-time, Long Term Care Client Coordinator. CVHHH is a full service, not-for-profit Visiting Nurse Association committed to providing high quality, medically-necessary home health and hospice care to all central Vermonters, regardless of a person’s ability to pay. In addition, we promote the general welfare of all central Vermonters with health promotion, long term care, and maternal-and-child health services.

Your Day-to-Day – Long Term Care Client Coordinator
- Creates, maintains, and updates schedules according to authorization and patients’ needs
- Receive and enter data in the EMR system for new and current patients and employees in an accurate and timely manner.
- Adjusts current schedule and notify appropriate staff members and clients daily in a courteous and professional manner
- Promotes excellent customer service to staff, clients, agencies, and other interested parties
- Creates spreadsheets, databases and reports as directed by the LTC Manager to better inform the department of programmatic trends and outcomes
- Working on site, from 8:00am-5:00pm

Requirements
- High School Graduate or GED
- 2 years of Administrative and computer experience, 6 months of data entry experience. Experience with scheduling software preferred.
- Valid Vermont drivers license.
