Adult Home Care Manager – $3000 relocation bonus

Your Day-to-Day – Adult Home Care Manager

  • Supervise and oversee the planning, organizing, and directing of patient care services, ensuring consistent delivery of high-value quality care to all patient populations served.
  • Lead, inspire and motivate others to provide exceptional care.
  • Ensuring that patient care is coordinated and managed appropriately in compliance with all federal and state rules and regulations.
  • Collaborates with others to coordinate services, streamline and standardize work flows and procedures.
  • Guide and maintain home care staff, including hiring, training, and coaching all team members
  • Participates in developing standards which insure safe and therapeutically effective service to patients and families while supporting clinical staff to practice at the top of their licenses.
  • Participates in coordinating agency’s home care services with services of other community, state, and federal agencies.
  • Evaluates periodically the service delivery system regarding regulations, professional practice, and the goals of the organization.  Understands Medicare regulations and works with Chief Operating Office (COO) and Chief Quality Officer (CQO) to assure adherence to regulations.
  • Collaborates with COO in program planning and budgeting, establishing administrative and clinical policies and practices relating to homecare services.
  • Collaborates with the CQO in developing the annual quality improvement plan goals for the homecare program.  Implements plan with the Home Care Clinical Team Leads and staff to achieve the goals.
  • Daily engagements with clients and family members to provide in-home high-quality care timely in prevention of rehospitalizations.
  • Ability to support staff toward the goal of optimal client care and satisfaction.

 Requirements  

  • Bachelors degree in nursing from an accredited RN program Minimum three (3) years of experience in  clinical management  or a combination of clinical supervision and post-acute care experience.  Experience in a Medicare home care environment preferred
  • Knowledge of CMS regulations, Vermont Home Care Rules, ability to comply with OASIS, OSHA, Infection Control, and HIPPA regulations
  • Valid VT RN License
  • Valid driver’s license
