Central Vermont Home Health & Hospice
We are looking for a Hospice & Palliative Care Registered Nurse to join our Hospice & Palliative Care team to provide one-on-one care for patients in their home setting.
Our Hospice & Palliative Care team serves individuals in need of pain and symptom management and comfort care, as well as those individuals nearing the end of life. You will work as part of an interdisciplinary team that also includes nursing aides, social workers, a bereavement coordinator, chaplain, and personal care attendants. The team is overseen by a clinical nurse manager.
Hospice supports patients with comprehensive care that meets all of a person’s needs. This includes medical care, emotional support, and education. As a member of our Hospice & Palliative Care team, you will become an important and trusted point person for patients and their families and will provide care wherever a person resides, including in assisted living facilities and nursing homes.
As a Hospice & Palliative Care Registered Nurse, you will provide the care that makes it possible for people to live wherever they choose. Your patients can wake up surrounded by familiar sights and sounds, and they can sleep in their own beds. - VT Licensed RN (BSN preferred)
- Minimum one year experience in medical/surgical environment
- Valid VT driver’s license and reliable vehicle with automobile insurance
- Maintain up-to-date, accurate, and timely electronic patient records
- Ability to work one on-call weekend per month
