Long-Term Care in Vermont

We know that support is often needed to make it possible to continue to live safely in your home. Sometimes, all that’s needed is help getting back on your feet after surgery or as you recover from an illness. Or, maybe you need help caring for a loved one who wants to continue to live at home and needs support with activities of daily living. Our Long-Term Care team provides in-home personal care and case management for adults living with disabilities and for older Vermonters who want to continue to live at home.

Long-Term Care Criteria

If you meet the following criteria, you may be eligible to receive Long-Term Care services:

  • You are a Vermont resident
  • You are at least 65 years old, or at least 18 years old with a physical disability
  • You meet the financial criteria
  • You meet the clinical criteria for nursing-home level of care, including having a physically-limiting condition resulting from a stroke or dementia, or something similar, or as a result of aging. Clinical criteria is determined by a nurse employed by VT Department of Aging & Independent Living (DAIL).

Help making breakfast, light housekeeping, and laundry.

Support from a CVHHH case manager to connect you with additional supports from CVHHH and community providers.

Assistance with bathing, mouth care, skin care, toileting, and dressing.

Conversation and companionship
Support of your physical and emotional wellbeing with games, writing letters, and taking walks.

To give loved ones rest and time to run errands, exercise, and visit friends.
