CVHHH Conducts First Community Visit Day

CVHHH conducted its first Community Visit Day in June. As part of this half-day event, representatives of some of the agency’s community health care partners joined CVHHH clinicians on home visits. The program was designed using a model developed by the VNA of Chittenden & Grand Isle Counties.

Participants included Jenna Corneille, Medical Social Worker at The Plainfield Health Center; Brian Aitchison, CVHHH Board Member and Vice President, Denis, Ricker & Brown/Hickok & Boardman Inc. in Montpelier; Patrice Knapp, Director of Quality Management at UVM Health Network-Central Vermont Medical Center; Eric Quintin, Case Manager at Washington County Mental Health Services; and Monika Morse, Assistant Director of Quality Operations at UVM Health Network-Central Vermont Medical Center Group Practices. Individuals were paired with a CVHHH clinician and went on two home visits each.

“One of our goals in initiating the Community Visit program was to invite our health care partners to experience a day in the life of a CVHHH clinician,” says Daniel L. Pudvah, Director of Development and Community Relations (pictured at left), who coordinated the day. “We hope that by following a nurse or therapist into the home, and seeing, firsthand, the level of care we provide, that our partners walk away with a deeper appreciation of the work our clinicians do every single day. Given the rapidly changing health reform environment, and the financial pressure on all providers, we believe it is important for our community partners to understand the breadth and depth of services we provide, and the impact of our care on our community.”

Over lunch and debrief with CVHHH CEO Sandy Rousse, Brian Aitchison reported: “The experience opened my eyes to the effect of CVHHH services on individuals on a daily basis.” Eric Quintin, added: “The visits made me realize how important it is that we work together in synergy to maximize the effectiveness of all sources of services provided.”

CVHHH plans to conduct more community visit days going forward.
