ContinueCare:
Everything you need to feel at home
ContinueCare provides services on
a long or short term basis to assist clients with their personal
care needs, housekeeping, shopping and errands, in addition to nursing
care, respite and companionship. ContinueCare services are provided
in minimum 4 hour visits in the client's home, a hospital or extended
care facility.
Services
are paid for privately by the client, their family, or private insurance.
The ContinueCare goal is to meet the needs of our clients and their
families, up to 24 hours a day, seven days a week, with flexibility
and compassion.
Although
ContinueCare is a private pay service, some Long Term Care insurance
providers pay up to 100% of costs. ContinueCare will work with you
and your insurance provider to determine your level of reimbursement
eligibility. Medicare-eligible services will be billed directly
to Medicare.
Gift
Certificates
Do you have a loved one, friend or neighbor in need of ContinueCare
services? Gift certificates in increments of 4 hour blocks are available
to assist someone you care about with light housekeeping, laundry,
shopping, cooking, errands and companionship. Click

Clients,

housekeeping, laundry, meal preparation, shopping, errands, and transportation. |

Clients
receive personal care assistance with bathing, grooming, toileting,
ambulation and transfers. |

Companionship |
| |
|
To
avoid loneliness and isolation, long term care staff members provide
loving companionship to clients who need someone with whom to talk
and socialize. Companionship provides an important link for frail
elders and others to feel part of the community. |

Nursing
Care |

Highly
trained and experienced nurses provide professional and compassionate
home-based care for people of all ages on a fee-for-service basis.
Care is available 24 hours a day, seven days a week. |

Care
Management |

Professional
Care Managers assist clients in gaining access to medical, social,
educational and other supportive services, and monitor the provision
of those services. They help clients respond to changes in their condition
or circumstances, allowing them to remain as independent as possible.
Care Managers also assist clients and their families assess and choose
an appropriate living environment. |

Respite
Care |

Knowing
that primary caregivers need a break from the everyday responsibility
of taking care of a loved one, respite care is available to allow
immediate family members "time off" to regenerate. |
