Currently, opportunities are available for the following positions:

Homecare Registered Nurse (download application)
Full-time (37.50 hours per week) Vermont licensed Registered Nurse needed to work in our Home Care program to provide skilled nursing care to individuals. As a Home Care Nurse you will be responsible for coordinating and providing high quality multidisciplinary skilled health services in a home setting to patients in the Central Vermont area.

Medical Social Worker (download application)
Full-time (37.50 hours per week) Medical Social Worker needed on our Hospice & Palliative Care Team. As a Medical Social Worker you will assess, plan, implement and evaluate direct care reflecting advanced knowledge, expertise and appropriate intervention in accordance with prescribed standards of the profession. Patients are a diversified cross section identified as either palliative or hospice, and are followed by members of the hospice & palliative care team.

Masters level graduate of a school of social work accredited by the council of Social Work Education or Certified Mental Health Counselor graduate.

1 year minimum of progressive clinical experience in a medically oriented setting.

Physical Therapist (download application)
Central Vermont Home Health & Hospice has a Full-time position available for a VT licensed physical therapist who is comfortable treating individuals of all ages.  Must have reliable transportation.  Services are provided in a home setting, the most realistic and functional environment possible. 

Occupational Therapist (download application)
Full-time / Part-time Vermont licensed Occupational Therapist needed in our Home Care Program. Must be able to assess, plan and evaluate direct care to our diversified clientele. Must have a minimum of one year of clinical OT experience and previous OTR experience in any homecare or healthcare setting.

PERSONAL CARE ATTENDANTS (PCA) (download application)
Caregivers needed in the community! CVHHH regularly has Personal Care Attendant (PCA) positions available throughout Washington County to help foster independence for frail and elderly residents. PCA's provide assistance with housekeeping, personal care, hygiene, transportation, and shopping. We provide services 365 days-a-year, 24 hours-a-day, so a wide variety of schedules are often available - from occasional on-call/per diem jobs to a regular full-time 37.5 hour week.
