Currently, opportunities are available for the following positions:

Hospice and Palliative Care Registered Nurse (download application)
Full-time (37.50 hours per week) Vermont licensed Registered Nurse is needed for our Palliative Care Program. As a Home Health Nurse you will be responsible for coordinating and providing high quality multidisciplinary skilled health services in a home setting to patients in the Central Vermont area.  You will also have the opportunity to provide care to high tech patients in the community with infusion pumps and other skilled care needs (training provided if needed). We are committed to providing high quality care that preserves patient dignity while helping to achieve the highest degree of recovery, comfort, and independence.

Requirements:  Vermont Licensed RN (BSN Preferred) with a minimum of one year of experience in a medical/surgical environment, valid driver’s license and reliable vehicle.

Homecare Registered Nurse (download application)

Licensed Nursing Assistant (LNA) (download application)

Part-time (Up to 10 hours per week) Vermont licensed LNA is needed to provide care in a home setting for two adults during evening hours between 8:30 p.m. to 9:45 p.m. Monday through Sunday.  Would consider any amount of days up to 7days/week. Compensation will include evening differential in addition to your hourly base rate.

Personal Care Attendant (PCA) (download application)

We are looking for energetic and caring individuals to provide personal care to adults who require assistance in the home.  Weekend and Evening hours available immediately with generous shift differentials.

Experience as a caregiver is a plus, but we will train the right person.  Must have valid Vermont Driver’s license and reliable vehicle.
